Activity/rest rhythm of depressed adolescents undergoing therapy: case studies (2005)

- Abstract: Abstract: Background: Disorders of circadian rhythms have been both reported in depressed childrenand adolescent studies. The aim of this study was to evaluate whether there is a relationship between 24h spectral power of the activity/rest rhythm and the clinical course of depression in adolescents. Methods: Six adolescents (14-17 years of age) were selected who suffered from Major Depressive Disorder (MDD), according to the DSM IV criteria, using Schedule for Affective Disorders and Schizophrenia for School Aged Children: Present and Lifetime Version (K-SADS-PL). Depressive symptoms were assessed using the Children's Depression Rating Scale-Revised (CDRS-R) and clinical evaluations. Locomotor activity was monitored over a period of thirteen consecutive weeks. Activity was measured during 10minute periods using wrist-worn activity monitors. Results: In five of the patients, the measurements of the 24h spectral power of activity/rest data correlated significantly with scores of the CDRS-R (p<0.05).One patient, who presented externally desynchronized rhythms during the depressive state, showed symptom relief when his activity/rest rhythms were entrained in the presence of novel physical and social cues. Limitations: A small sample was studied and no controls without antidepressive therapy were examined. Conclusions: The 24h spectral power of the activity/rest rhythm correlated significantly (negatively) with the clinical ratings of depression. The importance of social cues to entrain circadian rhythmicity and improve depressive symptoms was also observed in one patient
