Pain assessment during Peripherally Inserted Central Catheter placement in newborns (2010)

- Abstract: Background: The Peripherilly Inserted Central Catheter (PICC) is being used in critically il newborns at Neonatal Intensive Care Unit (NICU) since 1970s. PICCs offer a route for central venous acess via cannulation of a peripheral vein and are considered useful devices to deliver hyperosmolar solutions, drugs, and total parenteral nutrition. The catheter's use avoids repetitive venipunctures and decreases neonate's exposure to potentially painful procedures. However, few studies explored pain occurence in infants during the catheter´s placement.Objective: To evaluate pain scores in newborns undergoing PICC insertion.Methods: Descriptive study conducted in a tertiary-level NICU of a university teaching hospital at the city of São Paulo (Brazil). Study was approved by institutional review board and informed consent was obtained from parents. Data were collected prospectively from july to november 2007. Neonates with any neurologic impairment, genetic syndromes and congenital malformation were excluded. Pain was assessed by using the Premature Infant Pain Profile (PIPP) during venipuncture and during catheter progression.Results: A total of 28 newborns were enrolled. There was predominance of neonates born at 28-32 weeks of gestational age (15 - 53.5%); birth weight lower than 1500g, (20 - 71.6%); female gender (16 - 57.1%). Mean PIPP score during the first venipuncture was 7.79 (SD ±2.82): 14 (50%) newborns had PIPP scores ≥7, which indicates minor pain; 3 infants (10.7%) had scores ≥13, indicating moderate to severe pain. During catheter progression, mean painscore was 6.89 (SD ±3.03): 11 infants (39.2%) had PIPP scores ≥7; 2 neonates (7.1%) demonstrated scores ≥13. Six (21.4%) newborns received continuous analgesic or sedatives before the procedure. There are no protocolo or guidelines for pain control during PICC placement at the unit where data were collected.Conclusion: Results indicate that PICC placement is a painful procedure in different phases (puncture and insertion). For this reason, analgesic strategies, non-pharmacological and pharmacological, should be implemented during the procedure.
