Common variable immunodeficiency (CVID): clinical and immunological heterogeneity in brazilian patients (2003)

- Abstract: Introduction: Common variable immunodeficiency (CVID) is an incompletely defined syndrome characterized by defective antibody production. Probably because it has not been characterized into its many probably distinct syndromes, CVID is one of the most common primary immunodeficiency, often seen in adult patients. The aim of this study was to evaluate the immunologic status on CVID patients and to correlate laboratory data to clinical manifestations. Methods: 30 patients have been diagnosed according to ESID/PAGID criteria, and the laboratorial results were compared with 24 patients controls. Clinical symptoms were recorded and the following immunologic evaluation was performing for quantification of T (CD3) cells and its subsets (CD4, CD8), B (CD19), NK (CD16-56) cells was performed by flow cytometry. We also assessed lymphocyte proliferation after mitogen (PHA, Con-A) stimulation, and cytokine production after mitogen (PHA) stimulation. The main reported symptoms were compared with the laboratory data.Results: The analyses of the laboratory results of the patients showed a decrease of CD4/CD8 ratio in the majority of patients, due to a decrease on CD4 cells, or by an increase on CD8 cells. The number of CD19 cells was low, but NK cell numbers were not affected. The proliferative response to mitogens was preserved in a majority of patients, and we observed a tendency of increase of Th1 and a decrease on Th2 cytokine production. Theseresults did not correlate with worse clinical outcome. Conclusion: Changes in the immunological profile did not associated directly with clinical symptons, but demonstrate T cellular abnormalities involvement in most patients, suggesting a role in the pathogenesis in the disease
